Each year, the federal government requires that we survey our student population to identify how many military or federal employee dependents we serve. This number determines a large portion of our funding each year. Last year, Ridge View lost a significant amount of its funding due to an insufficient number of completed forms, so we ask that you help us improve on that result this year.
We will distribute Impact Aid forms of BLUE paper to all students during their third period class today, Tuesday, December 3rd. Students who are absent on this day will have another opportunity to receive the form when they return to school. In an effort to ensure that we have an adequate number of completed forms, teachers will collect these forms daily through Friday, December 13th, so please take a moment to complete this important survey, and return it to Ridge View High.
Instructions for completing the form are at the top of the form, so you will not need to refer to another page. However, if you have questions regarding the form, then please contact Patsy Smith at (803) 699-2999, extension 209. It is extremely important that each form be completed in ink, and includes the parents' signature, with a date after Monday, December 2nd, so please take care while completing this form. Unfortunately, the federal government requires that we have a completed for for every student; so, if you have more than one child who is a student in Richland School District Two, then you will need to submit the form separately for each child.
