In an effort to streamline the acceptance of sports physicals, completed physical packets can be emailed to the following email addresses:

Remember there are FOUR pages that need to be sent in: History Form, Physical Examination Form and both Richland 2 Sports Health Forms (one of which is the concussion acknowledgment as well as consent to treat). Packets can also be printed from RidgeViewSports.com.
Please convert to PDF before emailing packets in. CamScanner & Genius Scan are some examples of free mobile apps for PDF conversion.
Physical packets can also be turned into the Athletic Training Room or Coach Rosefield’s Office.
Do not slide Physical Packets under the door. Completed Physical Packets must be handed in or emailed to any of the above people.
All FOUR pages of the Packet must be completed. Partially completed physicals will not be accepted.
Packets must be dated after April 1, 2019 to be able to participate in Ridge View Sports for the 2019-2020 school year. Blank packets can be picked up in the Main Office, Athletic Training Room or Coach Rosefield’s Office.
