ATHLETICS: New Physical Packets and COVID-19 Waiver

The SCHSL has released a NEW physical packet to be able to participate in sports at the high school level.  Packets can be downloaded from RidgeViewSports.com.  Packets must be dated after April 1, 2020 to be able to participate in Ridge View Sports for the 2020-2021 school year.  All student-athletes must complete this COVID-19 Waiver before participation as well.

In an effort to streamline the acceptance of sports physicals, completed physical packets and COVID-19 Waivers should be emailed to the following email addresses:

All FOUR pages of the Sports Physical Packet must be completed along with the COVID-19 Waiver. Partially completed physicals will not be accepted.
